General Information

Purpose of the Appointment

A lawyer or insurance adjuster has requested an appointment for evaluation purposes.  This is NOT a standard medical appointment; Dr Vallentyne will not serve as a treating doctor.  Instead, this assessment will try to address the following questions:

  1. what injuries did you sustain in your accident?
  2. what is your current physical and functional status?
  3. are any further diagnostic investigations or treatments required?
  4. what is the future outlook for improvement or deterioration?

Duration of the Appointment

Your appointment will involve an interview followed by a physical examination.  The doctor will also review diagnostic investigations as well as clinical records in the preparation of the report.  Appointments typically last 1-2 hours in duration.

Language Preference

If you have difficulty speaking or understanding English, please inform your lawyer or insurance adjuster (in advance of your appointment) that you require an interpreter.

Attendees

Please feel free to bring one (1) person to attend the appointment with you.  Please note that young children are discouraged as disruptions will interfere with proper assessment. Please notify your lawyer or insurance adjuster if you require child care services.  A chaperone will be present at the appointment.

Parking

There is metered parking available on the street.

Late Arrival

Please leave adequate time to compensate for traffic congestion & weather conditions. The appointment will have to be rebooked if arrival is more than 30 minutes late.  Failure to arrive within 30 minutes of the scheduled appointment time will result in a late fee.

Cancellation

If unable to attend the above appointment, notice must be received in writing or e-mail 14 days prior to your appointment.  Contact your lawyer or insurance adjuster to cancel.  Failure to provide adequate notice of cancellation will result in a cancellation fee.
