What to Bring to Your Appointment

Please bring the following with you to your appointment:

1) List of your current medications

2) T-shirt & shorts (for the physical examination)

3) X-rays and imaging studies (films or CDs) as directed by your lawyer

4) Signed authorization form (see link under Patient Information).

If you wish, you may bring one family member or friend to you appointment.

If you have difficulty understanding or speaking English, please bring a translator (or have your lawyer arrange one).
